A significant limitation to interpreting results is the low response rate to the survey. The multidisciplinary professional interest group providing the foundation of our convenience sample included approximately 250 members. While it is not clear how many of this group were prescribers, notably only 45 respondents completed the survey, which limits generalizability of results.
